Extension is adding everything to one card

I think I set things up wrong. And I am not sure what is wrong.
If you look at image one. Under the dictionary section it is suppose to put the selected word in the dictionary section. But instead it is adding all the words I "don't know".How can I fix this?
I just need the word that is selected to go in the dictionary section. Not all the words I don't know from the sentence.
When I test the video, the same thing happens.Image 1:

Ah, thats what you mean. Sorry didn't get that the first time.Normaly words that get added to the "Word" field get deconjugated. Or in the drop down that is the "Unknown Word(s)" selection.

I did do that, but now it is coming up with the dictionary definitions. I just want how the dictionary sees it unconjugated.In the example below.
Missing: The card has the sentence with the missing word.
Word: the way it appears in the sentence. (conjugated)
Sentence: This is the sentence with the word being learned.
Picture: Picture
Sound: forvo
connection: Any memories I have with this word.
Dictionary: This is the unconjugated word. how you find it in the dictionary and if there is any hanja(kanji)
Meaning: the meaning of the word found in the dictionary in the target language.But, when every I use the migaku browser extension: not only all the words get add did but so do all the pictures for multiple words

Is this your own card type?
On the right side of the field where all the words get posted, click on the grey arrow. It should say "Definitions". If not, you have to change it to that.
